A healthcare provider is seeking a Patient Care Coordinator (Registered Nurse) to manage client care processes and ensure seamless coordination between patients, caregivers, and the care team. This role is ideal for a nurse with strong organizational and patient management skills.

• Conduct comprehensive client assessments and evaluate care needs
• Manage client intake, onboarding, and care planning with the nursing team
• Assign and schedule caregivers in line with regulatory requirements
• Oversee daily case coordination and monitor service delivery
• Support hospital-to-home care transitions for patients
• Ensure compliance with healthcare standards and policies

• Must be a Registered Nurse (RN)
• Relevant clinical experience is required
• Experience in home care is an added advantage
